Terms and Conditions of the Campaign:

• This campaign is exclusively for AIA Vitality members holding AIA Vitality integrated individual policies. Members must accumulate points and upgrade their status from Bronze to Silver 1 April 2025 and 30 June 2025.

• Members must accumulate 10,000 AIA Vitality points via the AIA+ application under the "AIA Vitality" menu during the campaign period to upgrade to Silver status and be eligible for the rewards.

• The AIA Vitality status referenced for this campaign is based on the benefit status displayed on the homepage of the AIA Vitality tab in the AIA+ application.

• Members who meet all requirements and conditions of the campaign are entitled to choose 3 rewards from the following options: 1) Boost Juice e-Voucher valued at 100 THB 2) Sizzler e-Voucher valued at 100 THB 3) GrabFood Code valued at 100 THB (applicable only to restaurants labeled "Healthy")

• Eligible members will see and be able to claim their e-Voucher rewards via the AIA+ applicatio in the "Privileges" menu, under "Special Reward / Campaign Reward," starting from the 10th of the following month after the member has met the campaign conditions. Members will receive the reward according to the following schedule:

• If rewards are not claimed by 31 July 2025, it will be considered a forfeiture of rights.

• Each AIA Vitality member is entitled to claim a maximum of 3 rewards throughout the campaign period. Once a reward is selected and confirmed through the system, it cannot be changed to another reward.

• The e-Vouchers received via the AIA+ application are valid until 31 July 2025.

• Members must claim AIA Vitality points in each category, including syncing fitness data through the AIA+ application, regularly and by 30 June 2025.

• Health devices or fitness applications compatible with the AIA+ application are provided by external companies unaffiliated with AIA. Therefore, AIA assumes no responsibility for the quality or services of these health devices or fitness applications connected to the AIA+ application.

• AIA reserves the right to disqualify participants without prior notice if any fraudulent activity or actions against the campaign’s objectives are detected.

• The AIA Vitality benefits are subject to AIA terms and conditions. AIA reserves the right to change and amend any of the terms and conditions which are available at the AIA+ application or at https://www.aia.co.th/en/health-wellness/vitality/rewards

Terms and Conditions for Boost Juice e-Voucher (Value: THB 100)

• The e-Voucher is valid only within the specified period.

• The e-Voucher can be used at Boost Juice Bars in Thailand, excluding airport branches and delivery services.

• The e-Voucher is valid for beverages only and cannot be used for food, bottled water, or premium merchandise.

• The e-Voucher cannot be exchanged for cash and is valid for single use only.

• The e-Voucher cannot be used for partial payments, and no refunds will be given for unused balances.

• If the purchase exceeds the e-Voucher value, the user must pay the remaining amount in cash or other accepted payment methods.

• The e-Voucher cannot be used in conjunction with other promotions.

• The e-Voucher cannot be replaced if lost, stolen, or damaged.

• Once the reward is claimed, the e-Voucher will appear under “My Coupons” in the “Used Coupons” section of the AIA+ app. Members can click “Redeem” to use the e-Voucher during checkout at the selected store. The e-Voucher is valid until 31 July 2025.

• To redeem, show the QR code from the AIA+ app to the staff. Only QR codes displayed directly from the AIA+ app are accepted; screenshots or other methods are not valid.

• Once the e-Voucher is successfully redeemed via the AIA+ app, it is considered fully utilized. AIA will not compensate or replace the e-Voucher in case of user errors.

• AIA reserves the right to change or cancel these terms and conditions with prior notice through the company’s communication channels.

• For questions or complaints about services, products, or offers provided by the partner service provider, please contact the service provider directly. In case of disputes, AIA’s decision is final.

Terms and Conditions for using Sizzler e-Voucher worth 100 THB:

• This e-Voucher is valid only within the specified time period.

• This e-Voucher is equivalent to cash in the specified amount.

• The e-Voucher can be used for dine-in and takeaway orders but cannot be used for delivery.

• There is no limit to the number of e-Vouchers used per receipt.

• This e-Voucher is valid at participating Sizzler branches (excluding Sizzler To Go and airport branches). Participating branches can be verified through this link: https://www.sizzler.co.th/th/all-branches

• After confirming the reward, the e-Voucher will appear under "My Coupons" in the "Used Coupons" menu. Members can click “Redeem” to use the e-Voucher during checkout at the selected store. The e-Voucher is valid until 31 July 2025.

• Please present the QR Code generated by the AIA+ application to staff to redeem the voucher. Only QR Codes displayed via the AIA+ application are valid (screenshots or other forms of display are not accepted).

• Successfully redeeming the voucher through the AIA+ app constitutes complete usage. AIA reserves the right not to compensate or replace vouchers in case of user error.

• AIA reserves the right to modify or cancel these terms and conditions with prior notification through the company's communication channels.

• For inquiries or complaints about services, products, or offers provided by partners, please contact the service provider directly. In the event of a dispute, AIA's decision is considered final.

Terms and Conditions for using GrabFood Code ("Healthy" labeled restaurants only), valued at 100 THB:

• This GrabFood Code is valid only within the specified period.

• The GrabFood Code is applicable exclusively for restaurants labeled "Healthy" and participating restaurants listed in the GrabFood section of the Grab application.

• This GrabFood Code is valid only for GrabFood services within Thailand.

• Limited to 1 code per order.

• This GrabFood Code cannot be used for "Scheduled Orders," "Dine-in," or "Self Pickup" services.

• This GrabFood Code cannot be combined with other Grab promotions or offers.

• The GrabFood Code cannot be applied to modify orders after they have been placed.

How to use GrabFood Code for restaurants labeled "Healthy" on the Grab app:

1. Select "Food" on the home screen of the Grab application.

2. Tap the "Healthy" icon on the GrabFood home screen.

3. Restaurants labeled "Healthy" will be displayed.

4. Select "Use Promotion" during the ordering process.

5. Enter the GrabFood Code obtained from the AIA+ application and click "Apply."

6. The applied GrabFood Code will be displayed in the ordering process. Proceed to confirm payment.
