Step 2 - Shop at Boots

Purchasing any products at participating Boots branches.The purchase price of 50 Baht is net price after deducting discounts.

Step 3 - Show discount barcode

Show discount barcode on your mobile phone to cashier before payment.

Step 1 - Book an appointment

Make an appointment with the participating Boots stores by phone. Walk-in allowed per pharmacist availability.

Step 2 - Present your e-Card

On your visit, present AIA Vitality membership card/ e-card along with national ID or passport to the pharmacist.

Step 3 - Scan QR code

Scan QR code, key in National ID or passport number to check your eligibility and receive free check up once per year.

Step 4 - Sign the form

Upon completion, you and the pharmacist must sign the form.

Step 5 - Upload your results

Scan QR code and upload health check results together with the pharmacist.

Step 6 - Submit results

Click OK to submit health check results. You will receive 50 Baht e-voucher to be spent at Boots.

Step 7 - Automatically earn points

Points will automatically be reflected on your point history.

Vitality Health Check is basic health screening comprised of Body Mass Index (BMI), Blood Pressure and Glucose measurements.
This privilege is only once per AIA Vitality Members per membership year.
Pharmacist will only conduct Vitality Health Check after checking your eligibility. Pharmacist will provide consultation based on health check up form and return health check form to you.
You may earn Vitality Points for BMI, Blood Pressure and glucose measurements and additional points for results that are within the healthy range, once every membership year. The measurements may be taken during the AIA Vitality Health Check, the AIA Vitality Nutrition Consultation, the AIA Vitality Fitness Assessment, an advanced screening or by self-submitting results from an external source, e.g. your general practitioner or doctor.

This e-Voucher can be use once per receipt at all branched of Boots except airport, Duty Fee shop, Paragon Food Hall and clearance branch. Exclusions are clearance products, medicine and cannot use with any other discount coupons.
