3 Simple Steps to Submit Your Health Check Documents
Visit any hospital, clinic, or AIA Vitality partner. You can check which health check activities qualify for AIA Vitality points below.
Submit your documents with just a few taps. The system will automatically review them and show one of these statuses:
✔ Approved: Your documents are correct, and points will be awarded.
⏳ Pending: Your documents are being reviewed.
❌ Rejected: Your documents don't meet the requirements. The reason will be provided.
You can check all document statuses anytime under the “Wellness Records” menu.
Once you submit your documents, the AIA team will verify them. If everything is accurate and complete, you will receive your Vitality points within 3–5 working days.

Tips for Submitting Documents That Get Approved Quickly
Make sure your health check documents are:
✔️ Clear and readable
✔️ Showing member name, healthcare provider name, and full test details
✔️ Not cropped or cut off
✔️ Submitted as multiple image files (.jpg or .png) or a single PDF (max 5 MB)
Requirements for Health Check or Vaccination Documents
• Hospital/clinic name or logo
• Member’s full name
• Details of the service or check-up
• Document date must be within the last 6 months and fall within your current membership year
Requirements for Fitness Assessment Documents
• Member’s full name
• Assessment date from a fitness partner (within the last 30 days)
• Test results and trainer’s signature
